The rules
An applicant 18 or older who passes the vision and knowledge tests is issued a regular Class E instruction permit.
The two tests are what stand between an untested applicant and a car, so both come before any permit.
A regular instruction permit lets the holder drive while a licensed driver 21 or older occupies the front seat with them.
An adult learner is still a learner, so the same supervision rule applies whatever the age on the application.
A regular instruction permit is valid for six months, and an expired one requires retaking both the vision and knowledge tests.
An expired permit is treated as untested rather than lapsed, which is why the tests are taken again.
The West Virginia knowledge examination has at least 25 questions, and 19 must be answered correctly to pass.
