Medical forms — Utah Permit Test

Medical forms on the Utah permit test — 4 rules, each with the reason behind it and a question on it while it is fresh.

The rules

A Utah applicant with a health condition that may affect driving is mailed a Functional Ability Evaluation form to take to a health care professional.

The form is how a condition becomes a decision rather than a rumour. A doctor writes down what you can still do.

If there is a health condition that may affect your driving, we will send you a Functional Ability Evaluation (FAE) form by mail to take to your HCP.

Utah Driver Handbook 2026-2027, p.21

A completed Functional Ability Evaluation form is returned to the Driver License Division for review.

The doctor does not decide who drives. The division does, and it needs the form back to decide anything.

The completed form is returned to the Driver License Division for review.

Utah Driver Handbook 2026-2027, p.21

A reviewed Utah health form may lead to license restrictions, skill and written knowledge testing, or in some cases a denial of the license.

There are three outcomes, not one. Most people keep driving with a restriction, which is why reporting a condition is worth doing.

If appropriate, a license to drive will be issued based on previously determined levels of driving risk and may result in license restrictions, skill and written knowledge testing (driver review) and in some cases, a denial of your license for safety reasons.

Utah Driver Handbook 2026-2027, p.21

A Utah driver has the final responsibility for knowing their own abilities and for driving safely.

Forms and hearings happen once. The judgment about whether to drive today is made by you, every day.

You have the final responsibility for knowing your abilities and for driving safely.

On the test

What does Utah send an applicant whose health may affect their driving?

A Functional Ability Evaluation form. The form is how a condition becomes a decision rather than a rumour. A doctor writes down what you can still do. Utah Driver Handbook 2026-2027, p.21: “If there is a health condition that may affect your driving, we will send you a Functional Ability Evaluation (FAE) form by mail to take to your HCP.”

How this works

Most practice tests hand you a bank of questions and leave the manual somewhere else. This one puts them together.

You read one law and the reason behind it, then answer a question on that law while it is still fresh. Get it right and you move on; get it wrong and you have found the gap while it is cheap to fix, not on the morning of the exam.

Learning one rule at a time, and proving it immediately, is what turns a manual you skimmed into something you actually know — and it makes studying feel like getting somewhere rather than like waiting to find out.

What it costs

  • One payment, once — not a subscription. Nothing renews and there is nothing to cancel, for an exam you sit one time.
  • Free to start: a 10-question test and the first section of cards. No sign-up and no account.
  • Paying opens the rest of the cards, unlimited full-length practice tests, and retests built from exactly the questions you got wrong.
  • Every level up to Prepared — 3 tests passed in a row.
  • Do the work, fail the real exam, and we refund you.
